• How do I make an appointment?

  If you would like to make an appointment with one of our practitioners, please contact our office at (623) 440-5053. Our team will guide you through the process and answer any questions you may have.

• What should I bring to my appointment?

  Please bring the following items to your first appointment:

  • Government-issued ID
  • A list of current medications

  • Any previous evaluations or medical records

  • Insurance card
     

  • Custody form (if applicable)
  • Consent from the other parent (if applicable)

  • School reports or IEP/504 Plans (if applicable)

• Can I stay with my child during their evaluation?

  At Peak Performance Institute, parental involvement is mandatory to ensure the safety, comfort, and success of each child during the evaluation process. Parents are required to remain in the lobby at all times while their child undergoes testing. This allows our clinicians to work with your child in a focused environment while ensuring that a parent is readily available if needed.

  Additionally, parents will be required to complete important questionnaires and forms during the session. This information is essential for developing a comprehensive understanding of your child’s development, behavior, and learning needs, enabling us to provide the most accurate and personalized recommendations.

  Your presence and participation are vital in supporting your child’s growth and ensuring the best possible evaluation experience.

• Will I receive a written report?

  All patients receive a comprehensive report detailing the findings, diagnoses (if applicable), and personalized recommendations. Reports are typically ready within 2–3 weeks after you meet with the doctor to discuss the results of the evaluation. 

• Do you accept insurance?

  We accept most major insurance plans. Please contact your insurance provider directly to confirm coverage for neuropsychological testing prior to your scheduled appointment.
